A large and growing in home Hospice Care in Bronx/Manhattan, NY is currently seeking an experienced Hospice Liaison Registered Nurse to join their team full-time! Check out the details below:

Pay: $54.65/hr-$68.00/hr based on experience and education

The Hospice Liaison RN will be responsible for the following duties:
- Collaborates with the Interdisciplinary team, coordinating comprehensive nursing services.
- Assess patients, discuss care options with families, and act as a key communicator between the hospital, patients, families, and other healthcare providers to ensure a smooth transition to end-of-life care.


REQUIREMENTS:
- 2 years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice
- Current NYS Registered Nurse license
- Baccalaureate Degree in Nursing
