An outpatient physicians office located in Kansas City, KS is currently seeking an experienced Clinic Registered Nurse or LPN to join their team full-time! Check out the details below:

Title: Population Health - RN or LPN
Job Type: Permanent / Direct Hire
Setting: Outpatient / Internal medicine / Triage
Location: Kansas City, KS (Onsite)
Schedule: Full-time, Mon-Fri, 8am-5pm
Pay: Competitive hourly pay based on experience plus full benefits

The Population Health is an essential member of a team responsible for the assessment, education, and care management of populations. Responsibilities include performing and documenting patient visits and monitoring conditions, conducting tests, and reporting information back to the primary care provider. The Population Health nurse communicates with patients and/or guardians to update on results, medications and treatment plans, and communicates further care options and support. The Population Health Nurse provides nursing expertise and is the liaison between the clinical care teams and the Population Health Team.

REQUIREMENTS:
- Active Kansas or compact Registered Nurse or Licensed Practical Nurse license
- At least one year of recent nursing experience working with adult population in a related setting: chronic care, disease management, patient education, triaging
- Up to date on vaccinations (covid not required)
- Bilingual Spanish/English is a plus
