Job Type: Permanent / Direct Hire
Setting: Home Health
Location: Blue Ash, OH
Schedule: Full-time, Mon – Fri, 8a – 4:30p
Pay: $55,000 - $63,000 per year plus a full benefit package


JOB DESCRIPTION:
The Assessor will conduct person-centered assessments to determine eligibility for various programs and services; provide education and support to individuals and their caregivers and ensuring that all documentation and compliance standards are met; work collaboratively with internal staff and external partners to advocate for clients and connect them with appropriate resources.


REQUIREMENTS:
- LSW license in good standing in Ohio
- At least one year of experience in medical social work, home health care, and/or geriatrics
- Good working knowledge and understanding of issues related to the elderly population highly desired
- Valid driver’s license with good record and reliable transportation
