A nonprofit healthcare organization in Topeka, KS is seeking a compassionate Hospice Admissions Nurse (RN) to join their team full-time! Check out the details below:

Title: Hospice Admissions Nurse (RN)
Job Type: Permanent / Direct Hire
Setting: Admissions - Hospice / Home Health
Location: Topeka, KS (travel to patient homes within approximately a 1-hour radius)
Schedule: Full-time, Monday-Friday, 8:00am-5:00pm with occasional on-call responsibilities
Compensation: $77,376-$89,572 annually, depending on experience, plus full benefits and a $3,000 sign-on bonus!

Job Duties:
• Complete hospice admission assessments and determine eligibility for services
• Educate patients and families on hospice philosophy, services, and plan of care
• Coordinate with physicians and interdisciplinary care teams to develop individualized care plans
• Arrange medications, equipment, and support services for new admissions
• Provide compassionate guidance and support to patients and families during the transition to hospice care
• Maintain accurate documentation and ensure compliance with hospice regulations

Qualifications:
• Active compact or Kansas RN license
• Graduate of an accredited nursing program
• Current CPR certification
• Valid driver's license and reliable transportation
• Strong assessment and communication skills
• Hospice, palliative care, home health, or case management experience preferred
