Title:Registered Nurse Care Manager - RN
Job Type: Permanent / Direct Hire
Setting: Family Medicine
Location: Kansas City, MO
Schedule: Full-Time, Monday-Friday 7:30am-5pm
Pay:$29.75-$43.15/hr based on experience

The Registered Nurse Care Manager will be responsible for but not limited to:
- Collaborates in the provision of care, treatment and services with the interdisciplinary team to meet patients’ needs and maintain continuity of care between disciplines, programs, services and with other organizations as needed
- Collaborates with the physician(s) and acts as liaison for the care coordination of the designated case load
- Follows policies and procedures in the assessment, planning, intervention, evaluation and documentation of patient and family care


REQUIREMENTS:
- Graduated from an accredited school of nursing
- Licensed to practice in the State of Missouri and/or Kansas.
- One-year professional medical-surgical experience, home health or hospice highly preferred
- BLS Certification
- Must possess a valid Driver’s License, automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard and reliable transportation
