Schedule: Full Time, Monday-Friday, 9a-5p
Pay: $125-160k based on experience

The Pharmacist will be responsible for the following duties:
- Provide palliative care for hospitalized patients and their families facing serious medical problems
- Provide expertise in medication use and monitoring, symptom management, and assessment of the risks and benefits of therapy and provision of continuity of care
- Order, interpret and evaluate appropriate imaging, diagnostics, and laboratory data, including but not limited to pharmacogenetic testing, as appropriate according to regulation and policy.

REQUIREMENTS:
- Active New York State pharmacy license in good standing
- 2 years of post-graduate residency/fellowship training, or equivalent experience in the care of patients facing serious medical problems preferred (e.g., oncology) or hospital/acute care experience
