Job Type: Permanent / Direct Hire
Setting: Primary Care Home Health
Location: Kansas City, KS
Schedule: Full-time, Monday-Friday 8:00am-5:00pm
Pay:$17 - $26.35/hr based on experience

The Community Health Worker will be responsible for but not limited to:
assist individuals and families in enrolling in public benefits such as Medicaid and the Affordable Care Act(ACA) marketplace, connecting them to critical resources, and providing financial literacy education. Community Health worker act as an advocate for the patient, acting a go-between between our providers and patients. If you have a passion for service and helping others


REQUIREMENTS:
- High school diploma or GED equivalent
- Certification or formal training as a Community Health worker highly preferred
