To welcome you back, we will award a reactivation award of 500 points redeemable in our Favorite Rewards program.
Update your credentials to begin working through us again and you’ll receive an extra 500 Rewards Points on completion of your first 40 hours. These bonus points will add onto the regular points you’ll earn with every hour of service through Favorite!
Program Rules:
Employees who have previously worked for Favorite but have not been paid within the last 90 days are considered to be a reactivation and are eligible to receive a 500 point reactivation bonus on completion of 40 qualifying service hours within a 60 day period. All other Favorite Rewards program rules apply.
Referral bonuses are in the spotlight and are designed with you in mind! A new and improved process helps you earn extra cash for sending your friends and colleagues to Favorite!
- Refer an RN or certain allied personnel, get $200 for every 50 hours your referral works, up to $1000
- Refer an LPN or certain allied personnel, get $50 for every 50 hours your referral works, up to $250
- Refer an CNA, MHT or HHA or certain allied personnel, get $20 for every 50 hours your referral works, up to $100

It’s easy - no application necessary! Beginning January 1st, 2007, just tell your friends and colleagues about Favorite and when they complete an application with us, we will ask them for their referral source. If they give us your name*, we will track their hours and pay you a bonus for every 50 hours they work, up to 250 hours within 180 days (6 months). * Make sure your referral knows your full name.

Program Rules:
- The new applicant must give us your name as their referral source upon hire.
- The referred employee must not have previously worked for Favorite and must perform 50 hours of billable service for Favorite (cancel pay, on-call and orientation do not count) for you to receive your first of a possible 5 bonus installments.
- The 250 hours must be completed within 180 days (6 months) for all installments to be issued.
- Bonuses will only be paid to active employees of Favorite who perform billable services for clients.
- Favorite reserves the right to modify or eliminate this program at any time.

You may designate a percentage of your earnings to be invested through pre-tax payroll deductions. (FICA and some state and local income taxes may still apply). Several different mutual fund investment options are available to you. Account statements will be mailed to you quarterly.

Housing Subsidies & Per Diem Allowances
If your travel assignment requires you to stay away from your home, we may be able to provide you with housing subsidies or per diem allowances if you qualify. Speak with a Travel Nurse Services Rep at 1-800-755-1411 or contact your local branch office to see if you qualify.
Favorite will provide private housing accommodations at one of our exciting travel assignments. We also help with the utilities. We work hard to make you comfortable so you can focus on your profession.
Many of our travel assignments include completion incentives up to $6,000, depending on the client and duration of the assignment.
Favorite can assist you with your travel expenses to ensure that you enjoy a comfortable journey to your travel assignment.
Continuing Education Reimbursement
Up to $100 per calendar year is offered for continuing education. A certificate of successful completion of the course and documentation of the amount paid for the course must be provided. The Tax Savings Plan is an employee benefit plan whereby you can redirect a portion of your pay to cover certain allowable child and dependent care expenses (e.g., day-care centers, home-base day-care, babysitters) on a tax-free basis.
As an employee of Favorite, you may contribute up to $5,000 per year under this program. You may join at time of hire or during an annual open enrollment period (usually early December).
Your contributions will be deducted from your pay and held in an account with the Tax Savings Plan administrator. When you incur an eligible expense, you will pay for it directly and then send a copy of the receipt and a claim form directly to the administrator, who will send you a tax free reimbursement. You can even choose to have reimbursements deposited into your bank account using direct deposit.

Favorite is concerned about your health and well-being and offers a free vaccination program to reduce the spread of the Hepatitis B virus.
You may enroll in this program upon hire or any other time during your employment with Favorite. The program consists of three separate inoculations given at varying intervals.
To receive this benefit you must be an active nursing employee and complete our mandatory training on the Universal Precautions and pass a written OSHA Blood borne Pathogen Exam.
Please contact your local branch now to complete the necessary training and shift requirements. Once completed, you should ask your branch representative to complete a Voucher Request for Hepatitis B Vaccination on your behalf. Upon receipt of verification that you have met the above requirements, you will receive instructions and a voucher for each inoculation.
Direct Deposit and Payroll Card
By enrolling in Direct Deposit or Payroll Card, your pay arrives in your account on the day you're scheduled to be paid and is immediately available for withdrawal. No longer are you forced to rely on the postal service to deliver your money, wait for the bank to credit your check, or pay the high fees of check cashing companies. Don't have a bank account? Not to worry. The Payroll Card is designed just for you! The Payroll Card is a service of Commerce Bank, made available through Favorite Healthcare Staffing, which will allow you to access your pay through ATMs throughout the country. There is no charge for enrollment or monthly maintenance of the account, but a small fee applies to withdrawals and purchases. Individuals who wish to enroll in this service should complete an application. Click here for a Payroll Card application form. Paid Liability Insurance & Workers’ Compensation Coverage
Favorite will provide you with professional liability insurance to cover errors or omissions while performing duties within the course and scope of your Favorite employment. To preserve this coverage, it is your responsibility to immediately report to Favorite any actual or alleged incident involving patient injury or death. In addition, you must cooperate with all provisions of the applicable medical professional liability insurance policy.
